SSI & doctors letter

Must one have a letter stating that you are not able to work if the medical records show that you are not able to work & in fact are not working due to IC. Has anyone else here on the ICN been able to get SSI without a doctors letter. The reason I'm asking this is because I am going to the VA hospital & not a private doctor. Another reason I'm asking this is because my husband & I are self-employed
