Social Security disability forms?

Do all states have the same forms to fill out? My forms just require a release signed to obtain my records from drs and hospitals along with all the questions you are familiar with I am sure. Do your forms suggest or require you get letters from your drs?

The forms I got from my SS office didn't have one single idea on how I could win my case. I took it upon myself to write and inform all of my drs that I was applying and asked for their help. Then, I made appts with them all and made sure that the paper work they had to send into SS was followed thru on. I lost on my first decision and won on my second.
Make sure and keep copies of EVERYTHING that you send them because if the stuff gets lost in the mail, it's your problem, not theirs.
