  • I could really do with some advice, please help...

    Hi everyone,

First off, I'm not sure I suffer from IC as my symptoms are much milder than everyone's on this board. I do, however, suffer from what I can only assume is recurrent cystitis which is driving me absolutely crazy. I hope someone will be able to take the time to read my long winded ramble below and give me some advice, although I know compared to everybody else here my condition isn't bad at all.

I'm 22 and I develop cystitis-like symptoms every pretty much without fail every time I have two or three days of sex. I never get cystitis apart from after sex and the sex itself isn't painful, just the aftermath.

I've been with my boyfriend for three years and have never slept with anybody else. It's a long distance relationship and when I first met him I wasn't working full time so I'd go up for perhaps a week at a time. We'd have sex every day, weren't overly stringent with cleaning, and I never had a hint of cystitis.

One visit, he had epididymitis, a condition he'd had every so often for a few years. We had sex anyway and that, as I remember, was the first time I got cystitis-like symptoms. They weren't that bad - I had an uncomfortable burning sensation in my urinary tract and urinary urgency although my bladder wasn't very full. I drank a lot of water and the symptoms vanished after a couple of hours.

Since then, the incidents of cystitis having been increasing. I now work full time so can only go to see him at the occasional weekend and I develop symptoms normally as I'm returning home. For a while they were very bad. I'd be in extreme discomfort (not exactly what I'd call 'pain', although it did hurt, but just this feeling of intense discomfort which for me is worse than pain), have urinary urgency, and on a couple of occasions blood and bits of skin in my urine. Antibiotics always helped very quickly. On less serious attacks I could luckily get away with just having a short course of those cystitis sachets from Boots and drinking a lot of water, and the symptoms would subside after a couple of days.

Lately the attacks haven't been so bad and are back to lasting just a few hours so long as I do nothing to exacerbate the situation. My boyfriend and I now wash carefully before and after sex with specially formulated washes (Nirena), I take cranberry tablets every day, I try to urinate straight after sex, all the normal things. But although the infections seem milder at the moment I still can't stop them from occurring no matter what I do. The sensations always feel to me to be around the entrance to my urethra and the urethral tract, not my actual bladder. I may be mistaken on this though.

My boyfriend and I plan to move in together soon and I'm so afraid that when we're together for longer periods and have more sex then the infection will really flare up and damage my bladder and possibly lead on to IC.

My boyfriend has also recently discovered that epididymitis can hang around even when no inflammation and pain is present unless an aggressive course of antibiotics is taken (which is what he's going to do). Do you think, therefore, I could have been developing cystitis due to infection in him being passed to me? It does seem strange that we are prone to these similar infections. He does a lot of weight lifting which I know is bad for epididymitis.

Also, do you think I could have the cystitis infection latent in me the whole time and sex makes it flare up and maybe I should go on a long course of antibiotics too to try to get rid of it once and for all?

Any sort of advice would be greatly appreciated. Again, I know that this sounds so tame compared to what you guys have to deal with but it really is making me depressed and anxious and I can't properly enjoy sex because cystitis is always on my mind.

    Thank you!

    Jenny

      Hi Jenny,

Anything that makes you depressed and anxious -like you obviously are over this- isn't tame. Sorry this is happening to you!

I really don't have any idea of what could be going on. Have you spoken to a doctor about this? My suggestion would be to see a doctor, ideally one you already have a good relationship with.

I hope you can get some answers and get to feeling better!

Thank you both so much for responding.

I'll go to the doctor, and hopefully she'll agree that a long course of antibiotics might help as you said, sailawaygrl. I'm glad you don't think it sounds as though I have IC, either! I'm just hoping it never progresses to that...

Hi, its hard to say at this point because there are a few things going on but I was told by a doctor years ago that strenuous sex can kinda bruise the urethra and give you that feeling afterwards.
Also, if some of the times you are getting bacterial infections which it sounds like you are then alot of doctors will prescribe an antibiotic like macrobid that you take once each time you have sex.

I have heard of women taking an antibiotic every time they have sex. Is it ok to do that, I'd have thought it would mess with your immune system and stuff or cause bacteria to become resistant.

No, I don't think so unless you're having sex everyday because then you'd be taking an antibiotic every day lol
I've been doing it for at least a couple of years and my doctors seem to think its fine. I think taking 3-5 days of antibiotics to get rid of an infection that occurs frequently would be worse. Not only that but getting frequent UTI's, like 3 or more in a year really wreaks havic on your poor bladder.

The two of you need to be working with a physician to track what is giving you infections, if it is infections. Ideally, it would be good of both of you could see the same urologist. Are you having cultures done when you have the infections? That should say whether you are having real infections and not mild injury from sexual activity.

I hope you are able to resolve this soon.

                Warm healing thoughts,

I have only just joined this board and have received some great advice and support, it's sooo nice when you log on and see that someone has taken their time to write to you...it really helps

I too only get cystitis when I have sex with my boyfriend....this has been happening with him for 20 years, in my 20's and 30's I used to get the bacterial cystitis almost every time we had sex, which I knew could be treated with antibiotics, but still excruciatingly painful.

                  BUT

The last 2 times I have had cystitis I have had no bacteria so antibiotics would not work.

Years ago my doctor said to me and I quote "try to be a little less violent in bed", how embarrassing that was. This doctor said I got cystitis from friction and bruising inside and I'm guessing that, that is the cause of the non bacterial cystitis and inflammation of the bladder I have now.

What would be great was if he could loose a inch or so from the length and half inch from the girth then I wouldn't keep getting it from him, we have laughed about him having circumcision or have me bite a bit off.

Luckily the first time I had this new strain (no bacteria) I was in Australia, they gave me an internal xray and said there was nothing serious and put me on anti inflammatory, amitriptyline, and anti-histamine and told me to get a cystoscopy when I returned to the UK.

This I had done and the surgeon said that my bladder was inflammed on one side and told me to continue taking the anti inflammatory, amitriptyline, and anti-histamines, after a few more weeks I got better.

My doctor then advised that I take 25mg of amitriptyline every day to try and prevent this happening again, so this is what I have been doing for the past 3.5 years.

BUT then I had a few heavy sessions (sorry hope its no tooooo much information lol) with the boyfriend and back again it came.

Ask your doc if you can take 25mg of amitriptyline every day as this has helped me during the past 3.5 years

I don't drink much but I have found if I force a lot of water down me that when I have to pee it doesn't hurt so much.

I am starting to see a pattern to when I get cystitis, regarding the positions we use during sex.....there is one in particular that seems to trigger the symptoms....the one from behind is the trigger for me, perhaps you may see a pattern and perhaps go a little more gentler in those positions.

I have also just been given some lubricant by my doctor to help prevent cystitis....she said that it is a lot better than KY, it's called Sylk...pronounced silk.

There is also the other stuff like:
                  peeing before and after
                  drink cranberry juice
                  drink lots of water

I hope this may help....at least you know there are others that get cystitis the same way as you...

                  xx
This is what I am taking and they work:
                  Ciprofloxacin 500mg (1 after jiggy)
Tramadol 50mg (1 or 2 pills) up to 4 times a day - Painkillers THAT ARE AMAZING

If you have interstitial cystitis, cranberry juice is probably the worst possible thing for you to drink. We call it the "acid bomb." The link to the latest IC diet food list is in my signature below.

That sounds really weird. Yeah, cranberry may be the worst, if you even suspect IC. Maybe try laying off of that for a while and see if that helps. Water, water, water. You really do need to talk with a uro and the suggestion of both of you having an appointment together is excellent. No one knows how IC develops, and never be embarrased about your talks with your doc. They have seen it and heard it all. And a lot of stuff much worse than your problem. Honesty with your doc is the best policy, you need to have a really good relationship, and if you don't like his/her bedside manner and how you are being treated, time to start shopping. Please keep everyone informed on how you make out. Jill, wife of Bob

Thanks to everyone else who has replied to me!

You're definitely right about me going to the doctor. I should have gone months ago. I'm going to try to get an appointment tomorrow with a doctor I like and I'll tell her the full story.

My boyfriend Phil actually went to the doctor today, who said it wasn't possible for the epididymitis to be passed on to me in any way. He also said Phil's epididymitis was so mild that it didn't require antibiotics. He thinks that the cystitis is caused because we have such long gaps in between seeing each other, often a month, that my body becomes unused to sex so when we do have sex bruising and irritation can occur which leads to the cystitis.

I suppose that might explain why for a year when we first met and were seeing each other more frequently I didn't get cystitis.

I will let you know what my doctor reckons tomorrow!

I have uti's almost monthly and sometimes it lingers a month. I have been trying to avoid antibiotics because then i usually get a yeast infection. I have been using antibiotics off and on but always have a burning sensation. Do you take them daily or only after sex.

                            I have an update

Had a cystoscopy and it turns out I have a narrow urethra and this is what is causing all the problems. It gets battered about

The specialist has prescribed Ciprofloxacin 500mg and has told me to take 1 after sex and I have done this and not had any problems so far, sometimes I take another 1 for another 2 days if I can still feel some pain and Tramadol sorts that out!!

I was on 8 Tramadol 50mg per day but I am down to 1 a day now, this is the ONLY painkiller that helps me, sometimes I take 2 if I feel the pain coming on......such a relief to know what has helped me and hope this helps any other ladies too

                            xxx
