I don't have an interstim, but have seen the device itself at an ICA convention several years ago. I don't know a lot about how it's implanted, but the wires are placed near the spine and the unit itself (a small "box") is implanted in the buttocks above the hip joint (I don't have one so someone else is better able to describe placement).
The unit itself should not be visible, but there will be a scar that will show. I have read comments by at least one very slender person that she had a visible bump where the unit is implanted.
The interstim isn't usually considered until all other treatments have been tried and failed.
